discharge information
For your convenience, patient discharge time is 1:00 p.m.
On discharge day, patients will receive specific written discharge instructions and an aftercare plan from their doctor. A health care professional will review the information with each patient, and patients will have the opportunity to ask any questions before signing the discharge form.
It is important that patients keep post-hospitalization appointments. Patients should take discharge instructions and aftercare plan with them to their doctor, clinic or program appointment.
return to work/school
Patients should let their doctor know before discharge if they need any type of return to work/school slip or insurance papers completed.
transportation back home
The patient or a family member are responsible for arranging transportation on discharge day. Please arrange to be picked up by 1:00 p.m. on the day of discharge.
